Hi, Is there a way to add a question to the register page and have it either allow or dis-allow your registration depending on the answer given?
Is there a way to use both recaptcha and Q&A?

Just edit the questions (think up your own unique ones) in the language files.

Your version of FluxBB appears to already have something like VSABR -- is it also possible to edit/customize the questions and language files in your code? Or would I need to also install the VSABR mod & edit the language files there? Is VSABR compatible with your version of FluxBB?

My version has js-protection and mathematical captcha.

Administration -> Plugins menu -> Security

If this is not enough for you, then additionally install Copyable captcha https://fluxbb.org/resources/mods/copyable-captcha/ (installed without changing the engine code, by simply copying files)

How does the JS protection work or what does it achieve?

Also, does the New reCaptcha mod work with your FluxBB as well?

My client is worried about spam so I want to have a lot of options to throw at him.

Part of the form is encoded on the server and then decoded on the client using js.

I have not tested it myself, but I saw on other sites this modification used with my version of FluxBB.
But I repeat again: all spammers aim to bypass ReCaptcha. And they bypass it using anti-captcha services. There is little sense from reCaptcha.

For additional protection, set up a start user group for him without the ability to leave links in messages/signature/profile. With automatic promotion of new members to members after N (usually 5) messages. Done in the admin panel - groups.

That's not really the question you ask for, but I installed this mod on my forum 9 months ago and only one bot passed during that time, ( I also was able to unban all the precedently ban domains ).
No questions asked, no trouble for the user and a terribly efficient spam barrier:
